Your report helps expose systemic problems and protect others. We protect the identity of those who report.

If there is an immediate threat to life or health, call 112 first.

Who is reporting

E.g. Alytus prison, police custody, social care home.

If you do not know the exact date, mention it in the description.

Type of violation

Describe the facts in order: who, when, where, who was involved, what were the consequences.

Witnesses, medical records, letters, CCTV cameras at the scene.

Where, when and what reply was received.

Publication

Without contact details we cannot follow up.
